Please make cheque payable to "Sun Life Trustee Co. Ltd." for scheme under Trust arrangement and "Sun Life Hong Kong Limited" for scheme under Policy arrangement. |

Mark the Employer Number on the back. |

Send the cheque together with a completed Contribution List to "Rm 706-712, 7/F, Sun Life Tower, 15 Canton Road, Tsimshatsui, Kowloon" at least 5 working days before the contribution due date. |

Alternatively, you may also hand in the cheque at one of our Financial Cafés. |
