Life Insurance Claim – Claimant’s Statement and CRS Self Certification Form

  • Life Insurance Claim – Claimant’s Statement: the Beneficiary(ies)/Claimant(s) should complete and sign their corresponding parts of the same copy
  • CRS Self Certification Form: each beneficiary(ies)/Claimant(s) should complete their own copy

  • Original death certificate
  • Policy contract (if unable to provide the contract, please provide a written statement to declare)
  • Copy of ID card/passport of deceased and the beneficiary(ies)
  • Relationship proof between the deceased and the beneficiary(ies) such as birth certificate/marriage certificate)
