COMPLETE THE CLAIM FORM AND COLLECT SUPPORTING DOCUMENTS

Fill out the following form
Critical Illness Benefit Claim Form

  • To be completed and signed by the Policy Owner and Insured

Critical Illness Attending Physician's Statement (submit the applicable one(s) only)

  • To be completed by the registered medical practitioner at your expense

Gather supporting documents
  • ID card copy of the Policy Owner
  • Original receipts and hospital statement (with name of patient, diagnosis and treatment details)
  • Copies of supporting documents such as discharge summary, referral letter, pathological and laboratory reports, etc.

Please submit the required documents within 90 days from date of diagnosis.

CLAIMS ASSESSMENT

After receiving the required documents, your claim will be processed within 7 working days*.

*Calculated from the date of claim received to the date of the first pending claim notice / claim result. The actual time required to handle claims may be dependent on the complexity of the claim, the completeness of the information submitted and the timeliness of responses from relevant parties (e.g., medical providers).

We may request additional information during the claims process. If you have any enquiries, please contact your advisor or our Client Service Centre.

PAYOUT

After the claim is processed, payment will be made via the method you have selected during the claim submission:

  • Pay to premium autopay bank account
  • Via Faster Payment System (FPS)
  • By cheque (mail to me)
  • By cheque (via advisor)
