COMPLETE THE CLAIM FORM AND COLLECT SUPPORTING DOCUMENTS

Fill out the following form

Life Insurance Claim – Claimant’s Statement and CRS Self Certification Form

  • Life Insurance Claim – Claimant’s Statement: the Beneficiary(ies)/Claimant(s) should complete and sign their corresponding parts of the same copy
  • CRS Self Certification Form: each beneficiary(ies)/Claimant(s) should complete their own copy

Gather supporting documents

  • Original death certificate
  • Policy contract (If unable to provide the policy contract, please fill in the Declaration of Loss Policy)
  • Copy of ID card/passport of deceased and the beneficiary(ies)
  • Relationship proof between the deceased and the claimant(s), e.g., birth certificate / marriage certificate). If relationship proof is not available (e.g., friends, fiancée, fiancé), please provide a copy of the identity document of the deceased and the claimant

CLAIMS ASSESSMENT

After receiving the required documents, your claim will be processed within 7 working days*.

*Calculated from the date of claim received to the date of the first pending claim notice / claim result. The actual time required to handle claims may be dependent on the complexity of the claim, the completeness of the information submitted and the timeliness of responses from relevant parties (e.g., medical providers).

We may request additional information during the claims process. If you have any enquiries, please contact your advisor or our Client Service Centre.

PAYOUT

After the claim is processed, payment will be made via the method you have selected during the claim submission:

  • Pay to premium autopay bank account
  • Via Faster Payment System (FPS)
  • By cheque (mail to me)
  • By cheque (via advisor)
