Fill out the following form

Bright Superb Health Medical Expense Claim Form

  • Part 1: to be completed and signed by the Policy Owner and Insured
  • Part 2: to be completed by the registered medical practitioner at the Claimant’s expense


Gather supporting documents

  • ID card copy of the Policy Owner
  • Original receipts and hospital statement (with name of patient, diagnosis and treatment details)
  • Copies of supporting documents such as discharge summary, referral letter, pathological and laboratory reports, etc.
