Accident Benefit Claim Form

  • Part 1: to be completed and signed by the Policy Owner and Insured (if submitting your claim via My Sun Life HK Client Digital Platforms, you don’t need to complete this part on a printed form)
  • Part 2: to be completed by the registered medical practitioner at the Claimant’s expense

Gather supporting documents

  • ID card copy of the Policy Owner
  • Original receipts and hospital statement (with name of patient, diagnosis and treatment details)
  • Copies of supporting documents such as discharge summary, referral letter, pathological and laboratory reports, etc.

We may request additional information during the claims process.

If you have submitted your claim via My Sun Life HK, you would receive a pending notification with instructions on the information required.

You may check your claim status in My Sun Life HK.

After the claim is processed, payment will be made via the method you have selected during the claim submission:

  • Pay to premium autopay bank account
  • Via Faster Payment System (FPS)
  • By cheque (mail to me)
  • By cheque (via advisor)

Track your Claim Status 

Claim status tracking is currently exclusive for Accident as well as Hospitalization & Surgical claims submitted via My Sun Life HK or paper forms.
For other types of claims, please contact your advisor or our Client Service Centre at (852) 2103 8928 for your claim status.

BY DEFAULT

You will receive a push notification once your claim status changed. Click on the notification and it will direct you to the claim details
