Disability Benefit Claims Form

  • Part 1: to be completed and signed by the Policy Owner and Insured
  • Part 2: to be completed by the attending doctor at your expense

  • Copy of sick leave certificates
  • Copy of ID card of Policy Owner (if you have never submitted to us)
  • Copies of supporting documents such as discharge summary, referral letter, pathological and laboratory reports, etc.
